Medical exams are required by the United States Citizenship and Immigration Services (USCIS) before you are approved for a Green Card or United States Citizenship. Only doctors designated by USCIS as Civil Surgeons can complete an immigration physical. Our Civil Surgoen have successfully performed thousands of these exams, followed strict guidelines established by the DSH, USCIS, and the Centers for Disease Control (CDC) to satisfy immigration exam requirements for patients of all ages.

1. I-693:  Immigration Physical

 

Our Price of $350 (Fee applies to Honululu, Houston and Costa Mesa locations ONLY) are inclusive of exam, blood,, and urine"

* Quantiferon (TB Test)

* RPR (Syphilis Test)

​*If there is no childhood immunization records available, additional 150.00*

 

Your Physical Exam will include at least 2 visits.  Your first visit will allow us to review your paperwork, medical history, vaccine requirements, and order the required tests--- CONSULTATION. The blood test results will take 3-7 days.  As soon as your blood test results are ready, will call you for a second visit which involves being seen by our Civil  Surgeon, who will thoughroughly review the laboratory results and conduct a physical exam to ensure that you have satisfied all requirements of the I-693 process.

 

2. N-648:  Medical Disability Waiver

 

***$850 for Medical Disability Waiver - (Add $50 per  applicant, if pay with Credit Card / Debit Card due to processing fees).  This fee covers for 3-5 visits-as needed to obtain all necessary medical history for any patients with underlying medical conditions that prevent them from studying for the English Naturalization Exam and Civic materials. 

 

​What is included in the Immigration Physical examination:

​

• Physical examination and completion of all the necessary paperwork

• Physical and mental evaluation

• A review of all available medical records

• Evaluation of other physical or mental abnormality, disease or disability

• Vaccinations and status verification

• Chest x-ray (if Quantiferon TB test positive)

 

Administration of vaccines and any other required services are available at additional cost.

 

What you MUST bring to your USCIS Medical Exam:

​

• Government-issued picture ID

• Vaccine/Immunization records

• Hospital or clinic records, including proof of prior treatment for tuberculosis or other infectious disease (like chicken pox)

• Prior chest x-rays and/or other diagnostic tests

• Any mental health records

• Do not fill out Form-693 as we will professionally type the form for you.
