      At First Point Urgent Care, our doctors are designated by the USCIS to perform immigration physicals exam and we have successfully helped thousands of patients with their immigration cases.

       Immigration applications are complex with difficult rules and requirements to follow.  Our clinic is fully prepared to work and assist you through every step of your immigration process, making this process stress-free and as smooth as possible.

     The most expensive part of the immigration physical is immunization which you received during childhood and can be difficult to obtain for some patients - But no worry, we can draw immunization titers and provide any required vaccines to meet USCIS requirements.

1. I-693:  Immigration Physical

 

***$289 Physical Exam/Form I-693 completion by the civil surgeon ONLY. Blood tests/vaccine cost are additional fees.

***$69 Consultation Fee per applicant to review needed tests and vaccines.  If decide to use our service & pay cash-will recieve this $69 as credit toward the total cost...

 

Your Physical Exam will include at least 2 visits.  Your First visit will allow us to review your paperwork and order the required tests (consultation).  Your second visit involves being seen by our Civil  Surgeon who will review the laboratory results and conduct a physical exam.  The Civil Surgeon will also ensure that you have satisfied all requirements of the I-693 process. 

 

2. N-648:  Medical Disability Waiver

 

***$689 for Medical Disability Waiver - (Add $50 per applicant, if pay with Credit Card / Debit Card due to processing fees).  This fee covers for 3-5 visits, as needed to obtain all necessary medical history for any patients with medical conditions that prevent them from studying for the English Naturalization Exam and Civic Materials. ​​

• The immigration medical examination entails a review of your medical history and a physical examination.​

• The Civil Surgeon will complete a comprehensive examination that will include a review of your medical history and a physical examination. The Civil Surgeon will also test for communicable diseases such as Tuberculosis, Syphilis, and Gonorrhea, depending on age, as required by the Centers for Disease Control and Prevention (CDC).  

• After the medical examination, the Civil Surgeon will complete Form I-693 and seal the form in an envelope for you to submit to USCIS.  Make sure you get a copy of the completed Form I-693 for your personal records before the Civil Surgeon seals the envelope.  USCIS will not accept the form if the envelope has been opened or altered.
