1. I-693:  Immigration Physical

 

***$289 for Physical Exam + $69 Consultation Fee per applicantYour Physical Exam will include at least 2 visits, which will allow us to review your paperwork and get started by running some tests.  Your follow-up visit involves being seen by our Civil Surgeon who will review your laboratory results and conduct a physical exam.The Civil Surgeon will also ensure that you have satisfied all requirements of the I-693 process. 

 

2. N-648:  Disability Waiver

 

***$689 for Disability Waiver - (Add $50 per applicant, if pay with Credit Card / Debit Card due to processing fees)This 

fee covers for 3-5 visits, as needed to obtain all necessary medical history. 

3. I-485:   Application For Green Card

    I-130:   Application For Alien Relative

   N-400:   Application For Naturalization

 

***$1,000 per applicant - (Add $50 per applicant, if pay with Credit Card / Debit Card due to processing fees).  This fee will cover at least 3 visits, which will allow us to review your paperwork

& to obtain all necessary attachments & supplements for your application.  Will provide Vietnamese translation service if needed for an additional fee...

During the Exam...

  • The immigration medical examination entails a review of your medical history and a physical examination.

  • The Civil Surgeon will complete a comprehensive examination that will include a review of your medical history and a physical examination. The Civil Surgeon will also test for communicable diseases such as Tuberculosis, Syphilis, and Gonorrhea, depending on age, as required by the Centers for Disease Control and Prevention (CDC).  

  •  

  • After the medical examination, the Civil Surgeon will complete Form I-693 and seal the form in an envelope for you to submit to USCIS.  Make sure you get a copy of the completed Form I- I-693 for your personal records before the Civil Surgeon seals the envelope.  USCIS will not accIept the form if the envelope has been opened or altered.

lab order to a lab near you, so when results are ready, you can see the

For I-693, (Immigration Physical)  

Bring The Followings To The Examination...

  • Government-issued photo identification:  Passport, Driver License, Birth Certificate...

  • Bring ALL of your vaccination or immunization records: DT,

  • DTP, DTaP, Td, Tdap, OPV, IPV, MMR, HIb, Hepatitis A & B, Varicella, Rotavirus  Pneumococcal, Meningococcal, Influenza, and COVID-19 as these are required by USCIS and varried by age. 

  • If you do not have all the vaccine records, we can draw titers to find out which vaccines you had
