           IMMIGRATION PHSYICAL or MEDICAL DISABILITY WAIVER

 

First Point Urgent Care hopes to be the clinic you trust to perform Immigration Physicals.  Medical examinations are required by the USCIS (United States Citizenship and Immigration Services) to obtain a US green card or US citizenship. Only doctors designated by the USCIS as civil surgeons can complete an immigration exam.  Dr. Tu Anh Luong and Dr Ryan Luong are authorized to perform immigration physicals and is listed on the USCIS website: https://www.uscis.gov/tools/find-a-civil-surgeon. We are open 7 days a week.

***Form I-693 USCIS medical report $289 ($319 credit card due to processing fees) Includes 1-3 visits and completion of Medical Certification

 

***Form N-648 Certification for those who are unable to take the citizenship test due to a medical disability. Cost $689 ($719 credit card due to processing fees). This covers for 3-5 visits as needed to obtain all necessary medical history 

What To Bring To The Examination

Bring the following to your medical examination:

  • Form I-693, Report of Medical Examination and Vaccination Record

  • Government-issued photo identification, such as a valid passport or driver’s license. If you are 14 years old or younger, bring identification that shows your name, date and place of birth and parent’s full name. Possible forms of identification include your birth certificate (with an English translation) or an affidavit.

  • Bring your vaccination or immunization record (such as DT, DTP, DTaP, Td, Tdap, OPV, IPV, MMR, Hib, hepatitis A, hepatitis B, varicella, pneumococcal influenza, rotavirus and meningococcal disease, and COVID-19)

  • Health insurance card. Many health insurance plans do not cover this immigration physical examination. However, it may cover vaccinations. Please call your insurance, as plan benefits vary

During the Exam

  • The immigration medical examination entails a review of your medical history and a physical examination.

  • The civil surgeon will complete a comprehensive examination that will include a review of your medical history and a physical examination. The civil surgeon will also test for communicable diseases such as tuberculosis, syphilis, and gonorrhea, depending on age, as required by the Centers for Disease Control and Prevention (CDC).  

  • After the immigration medical examination, the civil surgeon will complete Form I-693 and seal the form in an envelope for you to submit to USCIS. Make sure you get a copy of the completed Form I-693 for your personal records before the civil surgeon seals the envelope. USCIS will not accept the form if the envelope has been opened or altered.

 

LABORATORY TESTING

We collect and perform most tests in our office required for your medical report. Walk-in accepted 7 days a week

 

IMMUNIZATION

We advise on required vaccination, and can administer most required vaccines if needed

**Note**  While vaccination is covered by most insurance companies some plans will not cover vaccines if NOT provided by your PCP. It is your responsibility to know your benefits.

The following tests are additional charges and depending on age:

1. Syphillis Test                                                        $39

2. CT-GC  (Chlamydia/Gonorrhea) urine               $90

3. Quanti-Feron Gold TB Test:                               $139

(TB test and chest X-ray are good for 1 year)

(Add $30 if paid by credit card for credit card processing fees. These tests have to be done within 60 days of green card interview. The tests may not be transfered from health history or other records per the USCIS and CDC regulations)

For vaccines, our vaccine prices are very comparable. Patients can also choose to go to the Platte/Clay County/Jackson/Johnson County Health Department for most of the above shots.
